Patient Billing
We accept Visa, MasterCard, American Express, and Discover. Payment is due at the time of service unless otherwise arranged. For questions, call (480) 725-9188.
You are responsible for all fees, including those not covered by insurance. We send monthly statements. If insurance payment isn’t received within 4-6 weeks, please contact us. Any remaining balance is your responsibility, and payment plans can be set up in advance.

- Annual fees are due at enrollment and cover 12 consecutive months.
- Treatment and discounts do not roll over.
- Membership must be current to access services and savings.
- Non-refundable, non-transferable, and cannot be combined with other offers.
- Specialist referrals are not covered by this offer.
- This is not a dental plan, and savings do not apply to orthodontic services.
